All fees for services rendered are due on the date of services. So therefore, a fee of $400.00 is due on the date of all clinical assessments. A fee of $100.00 per hour is due for Individual Counseling Sessions, a fee of $80.00 per hour is due for Group Counseling Sessions, and a fee of $60.00 for each form that requires my completion.
In addition, a fee of $90.00 per hour (minimal one hour per month) is due for Clinical Supervision for Limited Licensed Social Workers and Limited Licensed Professional Counselors. I charge $200.00 to complete the SOS 258 Form for Driver’s License Restoration. I charge $100.00 for Couples’ Therapy. $90.00 for FMLA forms and $90.00 for Short-Term Disability Forms.
At present, I accept funds via PayPal and Zelle. There will be a $25.00 charge for PayPal payment due to their fees.
