All fees for services rendered are due on the date of services. So therefore, a fee of $250.00 is due on the date of your services (HPRP Clinical Assessment). A fee of $50.00 per hour is due for Individual Counseling Sessions, a fee of $70.00 per hour is due for Group Counseling Sessions, and a fee of $10.00 for each form that requires my completion. In addition, a fee of $50.00 per hour (minimal one hour per week) is due for Clinical Supervision for Limited Licensed Social Workers and Limited Licensed Professional Counselors. $155.00 for Drivers License Restoration. A fee of $150.00 per hour is due for all Consultations.
At present, I accept Blue Cross Blue Shield, cash, business checks, personal checks and Visa, MasterCard, Discover, and American Express, credit cards. Overdue accounts will be subjected to additional charges at the discretion of the owner.
